Each applicant must present a valid primary form of photo identification along with his or her application. If the applicant is unable to present a valid primary form of photo identification, two valid secondary forms of identification, one of which bears the applicant's signature.

Primary forms of identification:
  1. Driver's License
  2. Federal/State/County/City I.D.
  3. Student I.D.
  4. Military I.D.
  5. Employment Badge or Card
Secondary forms of identification:
  1. Voter's Registration Card
  2. Social Security Card
  3. Clinic or Health Insurance Card
  4. Hunting or Fishing License
  5. Credit Card
  6. Library Card

NOTE:  Birth Certificate can be issued if birth occurred anywhere in the state of Texas and a Death Certificate can only be issued if death occurred within Waller County.

Please mail in a written request that includes:

   -Full name of person on record
   -Date of Birth(Month, Day, Year)
   -Place of Birth(City, County, State)
   -Full name of Father
   -Full name of Mother(including maiden name)

Person requesting Birth/Death Certificate:

   -Address
   -Telephone Number
   -Relationship to person(must be individual or immediate family)
   -Purpose for obtaining this record
   -A copy of your valid Driver's License

FEE:

BIRTH:
   -$23.00 per Certified Copy

DEATH:
   -$21.00 for 1st Certified Copy
   -$4.00 for each additional Certified Copy

Our office does not take debit or credit as forms of payment. Forms of payment include cash, personal check, money order, or cashier's check.
